Understanding Epilepsy: Symptoms, Causes, and Risks

I’ve lived with epilepsy for most of my adult life. I’ve been through more than 26 medications, a Vagus Nerve Stimulator, consultations at multiple epilepsy centers, and more EEGs than I can count. So when I write about what epilepsy is, it’s not from a textbook — it’s because understanding the condition I live with every day has always been part of surviving it. I’ve found that the more clearly I understand the mechanics of what’s happening in my brain, the better I can communicate with my doctors, advocate for myself, and make sense of my own experience. That’s what I hope this post offers anyone else navigating the same road.

What Is Epilepsy?

Epilepsy is a neurological condition characterized by a tendency of the brain to release abnormal electrical discharges that disrupt normal brain activity. This abnormal electrical activity results in seizures, which are the main symptom of the condition. Seizures can range from short, unnoticed episodes to severe, convulsive movements, and they can last anywhere from a few seconds to several minutes.

Importantly, not every seizure indicates epilepsy. Epilepsy is defined by the presence of recurrent seizures that occur due to an underlying predisposition of the brain to produce abnormal electrical activity.

The Prevalence and Global Impact of Epilepsy

Epilepsy is one of the most common neurological conditions worldwide. An estimated 65 million people globally live with epilepsy (World Health Organization). In the United States alone, approximately 3.4 million Americans have epilepsy, with 150,000 new diagnoses each year (Epilepsy Foundation). Data from the CDC indicate that 1 in 26 people in the U.S. will develop epilepsy at some point in their lifetime (CDC Epilepsy).

Types of Seizures and Epilepsy Syndromes

Seizures fall into two primary categories. Focal seizures originate in one part of the brain and are classified as focal aware (the person remains conscious) or focal impaired awareness (the person loses consciousness or awareness). Generalized seizures affect both sides of the brain and often lead to loss of consciousness — this category includes tonic-clonic seizures, absence seizures, and myoclonic seizures. A third category, unknown-onset, is used when the starting point of a seizure cannot be determined from available information. Having lived with both focal and generalized seizures, I can tell you that the differences between them — in how they feel, how they present to others, and how they respond to treatment — are significant, and precision in classification matters.

Mortality and Risks

Up to 50,000 deaths occur each year in the United States due to epilepsy-related causes, including Sudden Unexpected Death in Epilepsy (SUDEP). Individuals with epilepsy are at 24 times greater risk of dying suddenly compared to the general population (Epilepsy Foundation). Seizure-related accidents — drowning, car crashes, and falls — also contribute to mortality (CDC). These are not abstract statistics for me. SUDEP is something I think about, and it’s something every person with epilepsy deserves honest, clear information about.

Causes of Epilepsy

In about 70% of epilepsy cases, the exact cause is unknown — this is called idiopathic epilepsy. In the remaining 30%, identifiable causes include head trauma, brain tumors, stroke, infections like meningitis or encephalitis, genetic conditions such as Dravet Syndrome, and metabolic disorders.

Seizure Triggers and Management

Common triggers include stress, hormonal changes, flashing lights in photosensitive epilepsy, and sleep deprivation. Tracking triggers through a seizure diary is one of the most practical tools available to anyone managing the condition. Knowing your own patterns doesn’t eliminate seizures, but it gives you something to work with — and it gives your doctor real data instead of approximations.

While there is no cure, advances in medical treatments and therapies continue to improve the quality of life for many people living with the condition. Understanding it clearly — what it is, what it isn’t, and what it can and can’t predict — is one of the most important things any patient or caregiver can do.
